MBBS degree & detail marks sheets of all professional examinations. PM&DC/PMC valid permanent registration Certificate. Copy of publications (if any) in approved journal from PM&DC/PMC/PubMed Indexed & HEC Domicile Certificate. Computerized National Identity Card (CNIC). Relevant subject/specialty related experience certificate from Public Sector & any Private Teaching Hospital recognized from PMC for Postgraduation Training. One passport size photograph duly attested from back side. Those having foreign qualifications are required to submit HEC/PMDC/PMC equivalency certificate.
